Participation in the progrm is completely voluntary and requires no monetary compensation on either the part of Futures Trading Journal or the applying registrant.

The form provided MUST be completed fully to be considered for this program; any missing information will automatically deny your participation in the program.

The registrant is responsible for providing the text information to be displayed and the necessary web-ready graphics in either .JPG or .GIF formats. No other formats will be accepted for graphics. Text may be submitted as either plain ascii text or in MS-Word format, no other text formats will be accpted.
